FormTip: Building Checklist Forms

This post provides a guide on how best to create a checklist using FormTab's FormTab Central form builder.

1. Use a segmented control field as this means you can require a positive or negative answer. 
   
   
   
   In most cases you probably want the options to be "yes" and "no".
   
   
   
   
   
2. Use the text block for long questions.
   
   
   
   Use short labels for the field names/labels. This ensures your CSV export has short label identifiers. 
   
   
   
   The end app user will see the full "block" of text.
   
   
   
   
   
3. Use the instructions field to provide instructions to the end user. 

   

   

The final printout won't show the text from this field.


4. Make use of your sections to make it easier for end users.

           

          Split your checklist into groups of 10-20 questions so that it's easier for app users to
navigate through the list and see where missing data may need to be completed.
